2024 Psychiatric Payment & Appointment Policy Summary
Credit Card Requirement:
A valid credit card must be on file before your initial evaluation. Payments are processed at the time of your appointment.
Insurance Reimbursement:
Services may be reimbursed by your insurance as an Out of Network benefit, but it's your responsibility to confirm coverage and submit necessary documentation, available through the patient portal. You are liable for all charges, even if your insurance denies payment.
Fee Schedule (Effective Jan. 1, 2024):
Initial Evaluation (60 mins): $355 USD
Follow-up (0–30 mins): $225 USD
Additional 15 mins: $100 USD
Cancellation Policy:
Cancellations must be made 72 business hours in advance.
Fees for late cancellations or no-shows:
Within 48–72 hours: $75 USD
Less than 48 hours or No-Show:
Initial Evaluation: $355 USD
Follow-up: Full appointment fee
Charges are waived if the slot is filled by another patient. Insurance does not cover missed appointment fees.
Additional Charges:
Paperwork (beyond attendance letters): $75 USD (adjusted based on time/complexity).
Time spent preparing/sending materials: Charged in 15-minute increments at regular service rates.
Financial Responsibility:
Patients are financially liable unless other arrangements are made (e.g., for minors).
Accounts overdue by 30 days are delinquent. After 90 days, services are terminated and accounts may be sent to collections.
Policy Changes and Record Requests:
Any financial changes must be discussed with the provider.
Accounts must be settled before records are transferred to another provider.
No Exceptions for Illness:
Missed appointments due to illness are still billable unless the time slot is filled.
